Patient Intake Form - Pet, Livestock & Equine

FILLL OUT ONCE PER ISSUE YOUR ANIMAL IS EXPERIENCING. NOT FOR RECHECKS. This required form allows us to understand and prepare for what your animal is experiencing prior to their appointment. Client Agreement Form

FILL OUT ONCE PER YEAR. This required form clearly defines the veterinary/client relationship and fee agreements. You will have to wait inside your vehicle and fill it out electronically if you do not have it finished prior to your appointment.

Credit Card Authorization Form

FILL OUT ONCE EVERY 6 MONTHS: This required form allows us to keep your card on file and charge it with your approval. You will have to wait inside your vehicle and fill it out electronically if you do not have it finished prior to your appointment.

Social Media Consent Form

FILL OUT ONCE PER YEAR. This form must be filled out and signed once per year. You will have to wait inside your vehicle and fill it out electronically if you do not have it finished prior to your appointment.

Equine Pre-Purchase Form

This required form allows us to understand what you know about the sale horse prior to our exam. You will have to wait inside your vehicle and fill it out electronically if you do not have it finished prior to your appointment.

Euthanasia Consent Form

This form must be filled out and signed before end of life care. Services cannot be provided until the form is completed.
